A 30-year-old man has met his untimely death after he was fatally crushed to death by a malfunctioning elevator.

The sad incident occurred at his luxury apartment home after Manhattan Medical Examiners determined Sam Waisbren died of blunt force injuries to his neck and torso.

He also suffered “mechanical asphyxia,” the ME found.

Waisbren was crushed by the elevator at the Manhattan Promenade tower on Third Avenue near East 25th Street just after 8:15 a.m., police said.

Surveillance video of the horrific accident obtained by The Post shows Waisbren tried to escape the packed lift as it plunged from the lobby into the shaft below, but he was crushed between the elevator car and the shaft wall.

On Friday, the city Department of Investigation said they were probing the death, but would not describe the focus of their investigation.

The DOI is responsible for “investigating and referring for criminal prosecution cases of fraud, corruption and other illegal activities by City employees, contractors and others who do business with the City,” according to its website.
